How to use Public Form Links

Set up a reusable public Smart Form that can be filled out via a link without logging into Jump.

Public Form Links let you create one evergreen Smart Form link and place it on your website, booking page, email signature, or standard client email. Submissions will appear in Form Activity under the "Needs review" designation.

With Public Form Links, you can collect initial information from clients at the start of onboarding and give them a consistent place to upload documents.


Setting up a Public Form Link

  1. Navigate to Forms > Templates.

  2. Either click on an existing template, or create a new template.

  3. In the panel that appears, click the Make public toggle to enable the public link. Then, click Copy link to get the shareable link. Click Save before exiting.

Once the link is created, you can send it out however you'd like. The link can be sent in an email, included in your email signature, sent via text message, or any other method.


Receiving and viewing responses

Once the client has received your link via your preferred delivery method:

  1. The client will see the form in their browser and fill it in just like a regular Smart Form.

  2. Once a form response is submitted, you'll find the responses in Forms > Form Activity. You can click the form to see the responses.

  3. In the panel that appears, you can choose to mark the form as reviewed, or associate it with a contact. You can also fill in missing information if necessary. From here, the form functions just like a standard Smart Form. Click Save when you're done reviewing the form.


Frequently Asked Questions (FAQs)

Can Public Form Links include prefilled client information?

No - Since these links are public, they cannot include prefilled information.

Can I enter a custom public-link name?

No - The link name is derived from the Smart Form template title.

Do Public Form Links expire?

No - and they cannot be configured to close after a specified number of responses. Public links will remain public until toggled off.
